Catering Event Permit form (One Day)
All caterers providing services in the city of Gardner must have a current Annual Catering License. Please use this form to register your events with the Board of Health.
In accordance with the provisions of 105 CMR 590.033 and Chapter 111, Sections 5 and 127A of the Massachusetts General Laws, and Board of Health Regulations.
No permit will be issued until processed by the City of Gardner Health Department. Event permit applications must be submitted 7 days prior to event.
